Certification Application
All exam applicants must meet the following requirements outlined below:
- Be at least 18 years of age
- Provide proof of current CPR certification
- Provide a copy of a U.S. government-issued ID, with your signature (driver's license, ID card or passport)
- Name on application must match government-issued ID used
- Statement of Proficiency

Medical assisting program graduate
Candidate Requirements
Candidates must be one of the following:
- A graduate of a medical assisting program in the United States within the last five years from a secondary, postsecondary or adult education program in a public school authorized by the Department of Education
- A graduate of a community college program or a postsecondary institution within the last five years accredited by an accreditation agency recognized by the U.S. Department of Education or approved by the Bureau for Private and Postsecondary Education
Required Documentation
- Certificate of completion or official transcript from an accredited medical assisting course with the application
- Instructor-signed injection/venipuncture statement of proficiency (PDF)
Currently employed medical assistant
Candidate Requirements
- Currently employed as a medical assistant under the supervision of a physician (MD/DO/DPM)
Required Documentation
- Verification of employment (e.g., current pay stub)
- Employer-signed injection/venipuncture statement of proficiency (PDF)
Previously employed medical assistant
Candidate Requirements
- At least two years of previous employment comparable to full time (4,160 hours) within the last five years as a medical assistant in the United States
Required Documentation
- Statement from previous employer(s) verifying number of hours worked in the last five years
- Employer-signed injection/venipuncture statement of proficiency (PDF)
Currently employed medical assisting instructor
Candidate Requirements
- Current employment as a medical assisting instructor in an accredited institution in the United States
Required Documentation
- Verification of employment (e.g, current pay stub)
- Statement of proficiency signed by the medical program director (PDF)
United States Military
Candidate Requirements
- Member of the United States military with relevant training or either separated from the military within the last year or served in that capacity for at least two of the previous five years
Required Documentation
- Copy of DD214 or proof of current military employment
- Commanding officer-signed injection/venipuncture statement of proficiency (PDF)
